The project team must define the evaluation plan during the design phase to ensure consistency, prevent errors, and maintain focus to aid the project’s success (Dawson, 2019). An evaluation plan allows the team to center on meeting goals and outcomes and keeping budget, costs, timelines, and resources within an established domain. In addition, it emphasizes the metrics to determine successes, failures, next steps, and team performance in achieving project outcomes (Harris et al., 2020).
My scholarly project involves implementing a validated assessment tool to screen elderly patients outside the intensive care unit (ICU) for delirium, a significant but often unrecognized risk factor for adverse patient safety issues. The primary measurable outcome will show the percentage of patients screening positive for delirium using the tool. A measurable process outcome is the percentage of screenings completed on admission and each shift for patients meeting eligibility criteria. Another process measure would be the percentage of positively screened patients referred to providers during this project’s 8-12 week timeframe.
The electronic medical record (EMR) will be the primary source for outcomes evaluation. I will review the EMR of patients meeting eligibility requirements for an admission screening and continuous screening at least once per shift for up to 5 days post admission. I will also check the provider notification of the patient’s positive screening and the outcome of this notification. Hopefully, this will provide the needed data points to proceed with the analysis of results.
