The questions are a way to instill and encourage a culture of improvement within the organization. If the members of the organization are continuously looking for ways to improve their quality of work, it is easier for the organization to implement measures to help with CQI. By using a planned, structured approach, CQI initiatives evaluate the practice’s strengths and weaknesses and update the practice continues to reach the vision and mission set by the organization. The tools used in CQI enable the members of the organization to assess their quality of work and to improve their patient care (National Learning Consortium, NLC, 2013). While evidence-based practices are usually published, CQI does not need to be published and is more of a strategy to implement than a practice. It is specific to the site that is using CQI and every site will have its own approach.
On the unit that I am employed, our manager uses a CQI approach to reduce the occurrences of hospital-acquired infections, such as CLABSI, CAUTI, and VAP. Whenever there is an occurrence of these on a patient, the unit has a huddle to discuss what was done, what could be improved and how does it not happen again. This has been very effective in the unit’s approach the patient care because the number of these occurrences have been lower significantly. The nursing assistants help a lot by always willing to empty Foleys, do Foley care, and cleaning the patient. The housekeeping staff also is aware of this culture and do their best to clean any messes as quickly and efficiently as possible. Because there is a culture against hospital-acquired infections on the unit, all the staff on the unit, including the nursing assistants and housekeeping, works towards preventing these infections from occurring.

The goal of CQI may include improvement of operations, outcomes, systems processes, improved work environment, or regulatory compliance (Brown & Marshall, 2008). Although risk assessment and pressure ulcer prevention efforts had been in place for many years, it continues to be a national health care problem. CQI initiatives that are implemented creates a clinical culture of pressure ulcer prevention, resulting in improved patient outcomes and cost savings. At my facility, we utilize a turning clock so that patients are turned to the appropriate position at the appropriate time. We also have positioning pads that are air filled and allow for shearing prevention and off-loading of bony prominences. In addition to these methods, a padded preventative dressing can also be applied to areas of concern along with moisture barrier cream. References
