The Rand Schrader Clinic provides AIDS/HIV testing, treatment, education, and prevention, information and referral, outreach, counseling, and STD testing. Being onsite and seeing how care is provided to the vulnerable population of my community will grow my understanding of clinic care and public health nursing. In the clinic, I propose to conduct a community needs assessment and interview and collect data in the investigation of disease outbreaks within the HIV community. I also want to interview patients of the clinic and survey them on ideas for care experience to improve. I want to follow the national initiative Ending the HIV Epidemic (EHE) plan that has been adopted by LACDPH Division of HIV and STD programs to better guide my ideas of care improvement projects. My goal for the entirety of my practicum is to have all newly diagnosed HIV positive patients at the Rand Schrader Clinic invited for a Partner Services interview within 7 days of their diagnosis. This interview will help the patients engage in HIV care and ensure that any sex or needle-sharing partner is tested for HIV and linked to PrEP (Pre-exposure Prophylaxis) and/or SSPs (Syringe Support Service Programs) to prevent forward transmission. Addressing structural and individual-level barriers to staying in care for their diagnosis will be a focus of patient interview and surveys implemented for each office visit. My preceptor Belinda manages her team of nurses, attends, and runs meetings and works closely with physicians and other care team members to provide optimal care to the patients that visit the clinic. My proposed project and other tasks at the clinic align with many of the PHN standards. As the weeks progress and my time at Rand Schrader continues, I will be involved in greatly in Standard 5: Implementation. This standard involves coordination of care, health teaching and health promotion, and consultation. As I create an interview template for the patients at the clinic, I will base my questions on Evidence-Based Practice and Research (Standard 9) as well as Resource Utilization (Standard 15). Advocacy (Standard 17) is a major standard of nursing practice and my work with the patient population at the clinic will focus heavily on this aspect of PHN.
