Record any major illnesses or diseases they had (even if this did not cause their death), and their age at death. FYI- the hhs.gov site is another popular one. Their site creates XML files when saved directly, so the easiest way to submit what you create from this site is as follows- click on the tab to view your diagram and table, click on the tab to print (or use Control p or Command p), then print to pdf. (FYI- you will need adobe acrobat installed for this function. It is available free online if your laptop currently doesn’t have it). You can save the pdf file and submit that to me via blackboard. Another option is to take a simple screenshot (Alt + Print Screen or Command + Shift + 3) and upload that to blackboard. Please do not spend too much time attempting to get the tree to look “just right”. You can even upload a cellphone picture… The point of the assignment is to go through the steps of creating a family history tree, and more importantly critically analyzing your risks. Look for any common family patterns (cancer, heart disease, diabetes, depression) and assess your familial risk. If you do not know some of the information, do your best to find out. If it is impossible to find out, simply account for this lack of knowledge within your paper and dig deeper into your behavioral risks as an alternative. Next, I would like you to observe your own health behaviors, positive and negative, and critically assess/analyze how your personal health behaviors may influence your risk factors (i.e. are they helping mitigate risk or are they increasing risk). FYI- this portion of the assignment should be just as detailed as your family history. Refer to the rubric criteria for detailed expectations. Instructions- Write a 3-4 page (minimum) paper addressing the following: 1. An examination, analysis, and discussion of your familial risks. 2. An examination, analysis, and discussion of your behavioral risks. 3. Identify and discuss at least two prevention techniques that can be implemented to mitigate your specific risks. 4. What specific habits should you adopt or change to improve your chance for success, and what specific strategies will help to that end? 5. Submit both your completed family tree and your written paper. They can be submitted separately or as a single document. All written submissions should be academically sound and free of typos or grammatical errors. Submissions must follow current APA requirements, with 1-inch margins, typed in 12-point Times New Roman font, double spaced, and include an appropriate title page, running heads, page numbers, correctly formatted in-text citations, and references where appropriate, and be uploaded as .doc or .docx documents.
